Job Description
JobPrism is looking for Advanced Clinical Practitioners in Hillswick.
You will manage undifferentiated presentations and deliver advanced assessment within primary care pathways.
Flexible sessions available up to 4 days per week, with regular clinics or short-term cover.
Role Snapshot
• ACP-led clinics supporting access, triage and safe ongoing management.
Key Responsibilities
• Undertake advanced history taking, examination and clinical decision-making
• Assess and manage acute presentations with appropriate investigations
• Prescribe within scope or work under agreed PGDs/prescribing support
• Coordinate care plans for complex patients with multidisciplinary teams
• Document consultations clearly with robust coding and safety-netting
• Escalate appropriately and contribute to shared clinical governance
Working Pattern
• Temporary, contract and locum options available
• Up to 4 days per week with flexible session structures
• Face-to-face clinics with some remote work possible if agreed
Requirements
• Registered clinician with ACP-level qualification or equivalent experience
• Recent UK primary care or urgent care experience managing undifferentiated patients
• Strong knowledge of local referral pathways and risk management
• Competent using EMIS or SystmOne and maintaining accurate coding
• Current mandatory training including safeguarding
