Job Description

JobPrism is looking for Advanced Clinical Practitioners to enhance same-day access in Hitchin.

You will manage undifferentiated presentations and deliver advanced assessment clinics.

Up to 4 days a week available with flexibility across sessions.

Role Snapshot
• ACP role supporting urgent care, triage, and complex patient reviews within primary care.

Key Responsibilities
• Assess, diagnose, and manage undifferentiated and complex presentations
• Provide advanced clinical decision-making and treatment planning
• Prescribe and/or request diagnostics within scope and local policy
• Work with the duty team to support triage and patient flow
• Identify red flags and escalate to the appropriate clinician or service
• Coordinate onward referral and follow-up with robust safety-netting
• Maintain accurate records, coding, and outcome documentation
• Support clinical governance, audits, and pathway adherence

Working Pattern
• Up to 4 days per week
• Flexible sessions including half-days where needed
• Session mix agreed to match practice demand

Requirements
• Relevant professional registration and ACP level practice experience
• Advanced clinical assessment skills and evidence of competency framework alignment
• Independent Prescriber preferred
• Experience in primary care, urgent care, or equivalent setting
• Strong risk management and escalation judgement
• Enhanced DBS and UK right to work
